I always put different sections in different groups, other than that I don't think it's worth the effort.

its not much effort to do it while your working, not something that should really be charged for.....if a designer wants to call themselves professional then it should be expected to be done.

This is where communication is key. All you need to do is say the word "inefficient" and you'd be surprised how their attention to what you have to say is undivided.
When I worked FT with an agency, one of their designers had the habit of things being splashed all over the place ALONG with random screenshots that didn't need to be in there (and yes, this were visible, until you made other layers that were over it invisible.)
You just need to communicate that this messy PSD is counter-productive. They'll change that very quick.

I consider it like naming your variables in programming. It is not physically required but anyone who knows what they are doing should do it.

Being a coder, I've charged more in the past if a client gave me .PSD's that weren't organized at all.

It's definitely good practice for you to keep your PSDs organized. I try to keep mine organized for working on the design in general. It makes things easier for me, and for any future edits someone else will need to make.

I must say I never organise my psds at all. Reason being I don't intend to sell PSDs, consider it annoying and puts off my flow if I am doing it while I work. Also when I code I tend to just code from an image instead of a psd (sometimes necessary).
When I have bought PSDs before I also didn't find it helpful. All the groups just made it more frustrating to find things which right clicking on objects.
I would say if it was for a client that specifically requested I would do it for them but even when I have worked for companies and created psds for them, they haven't minded. It is one reason why I don't both to submit psds to themeforest though.

I name each layer and organize different page elements into folders/subfolders all while I design. I rarely merge things together and when I do, I will sometimes have a hidden version thats unmerged. Then when I am going to hand the design over I will quickly go through it to make sure its nice and organized.
While I don't see this as an extra thing to charge for, usually its the talented and more professional designers who keep their files organized...so then again I guess you might be paying more anyways. I think this is similar to what enigma said.
