I'll try to find a solution for the missing avatars, but the quickest thing to do would be to re-upload.

Wouldn't Admincp > Maintenance > Update Counters > Rebuild Custom Avatar Thumbnails & Rebuild Avatar Thumbnails work? Minus; Include automatic JavaScript redirect to next page?
Not sure if the above will work or not, but I just re-uploaded mine.

No such option in the admincp, perhaps that is a feature of a newer version.

Ah okay, yeah, I'm used to 3.8.2, 3.8.4 and 4.0 ... Was worth a shot though! Perhaps after upgrade, it could be done then...I figured, since everything is still stored in the database, it's there, just needs "reminded/rebuilt". Only the urls were goofed for a little while.
